Our Medical Food Box Program provides nutritious, medically tailored food to individuals living with diet-related illnesses like diabetes. In partnership with healthcare providers, we connect patients to the healthy food they need to manage their conditions, support long-term wellness, and improve their quality of life.

At the heart of this work is FrescaZona, our community-based food hub, where a dedicated team of Community Health Workers supports families beyond the box. In addition to distributing medical food boxes, our team helps individuals access critical nutrition resources such as SNAP application assistance, Double Up Food Bucks eligible CSA produce boxes, and evidence-based health interventions like Family Check-Up 4 Health. This integrated approach ensures that families are not only receiving food today, but are building the tools and support systems needed for lasting health.

With one of our major funders unable to continue support this year, we are facing a critical gap in sustaining this work. We are turning to our community to help ensure that this essential connection between healthcare and nutrition continues.
